Advanced Surgical Concept (ASC)

Advanced Surgical Concepts is the onsite out-patient surgery center located in the same facility as Comprehensive Pain Management.  ASC Provides our patients with an inviting atmosphere, a caring staff and a state of the art facility.  Advanced Surgical Concepts is state licensed and accredited which allows us to provide specialized care and excellent service for each of our patients.

Please print the following acknowledgment forms and bring it with you to your appointment.

• Acknowledgment of Receipt of Information Financial Interest and Advance Directive
• Acknowledgment of Receipt of Information Privacy Practices
• Consent for Procedure

Your doctor will give orders for you to be scheduled for a procedure at Advanced Surgical Concepts. The surgery center is located at 9118 Bluebonnet Centre Blvd., Baton Rouge LA 70809.

In order to make the process smoother, we want to make you aware of the
following:

1. A representative from our office will be contacting you to discuss required forms and financial requirements. Please make sure we have your updated phone so contact can be made with you.
2. There are several forms that need to be read and signed prior to your procedure date. You may fax these forms to 225-368-2275 or bring signed forms the day of procedure:
   • Pre-procedure instructions.
   • Acknowledgement of Notices for Patient Bill of Rights
   • Submission and Investigation of Grievances
   • Disclosure of Financial Interest and Advanced Directives
3. Financial arrangements. - You will have TWO separate fees:
   • One fee for the ambulatory surgery center for rendering of services.
   • One fee for the professional portion for services provided by the Physician

   Both fees are estimates and due at time of service. Benefits are verified for the surgery center. You will be notified for the amount you are responsible in regards to the facility fee. This fee is due at the time of Service.

   Please make arrangements with your physician for the professional fee.

4. Bring your license and insurance card.
